For patients: How to request your records

Request records for personal use or continuing care

Patients who need to access their records for personal use or continuing care can easily access or request records via MyChart, our dedicated patient portal. MyChart allows you to download, save, print or request copies of self-downloadable health records, including:

How to print or save records in MyChart

  1. In the MyChart web browser, navigate to your desired visit summary, medication list, immunization record or other downloadable health record.
  2. In the upper right corner of the page, you’ll see a printer icon, a gear icon or 3-dot icon, depending on the web browser you choose.
  3. Click the icon and follow the prompts to print or save your record as a PDF.

How to request additional records

To request records that cannot be self-downloaded, log in to MyChart and select Request Medical Record on the main MyChart menu. Fill out the records request form completely and follow the prompts to submit the request.

You can also request your records through the Sharing Hub in MyChart. Click Sharing Hub from the main menu, choose Request Formal Copy of Health Record, then follow the prompts to request your records.

Patients who visit our St. Francis Campus can log in to MyChart here:

You can also request medical records for personal use or continuing care by completing our Patient-Directed Request form and returning it to Health Information Management.
